When filling in form for medication not sure which to tick I have a pivotal device which alarms and dispense my medication without the device I would forget it or overdose accidentally or deliberately as can get v depressed if u could explain it in easy to understand way

I would explain it in your answer to question 5 - Managing Treatments. Remember it's not that you use the device that will score but why you need to use it and that you would not take your medication successfully without it.
