In case you are not aware, your real name appears to be showing in the forum, if you want to change this then follow the instructions in the following FAQ
My full name is showing, how can I stop it?
If you have not made the initial call to start your PIP claim then you will be asked whether you want your DLA information included. They will not ask about your ESA.
Our recommendation is to include any evidence that you want to be considered with your PIP2, that way you can reference it in your answers to the questions on the form and can obviously ensure that the information is actually looked at by those assessing you.
