Assessors will vary their script to some extent based on what they have been told during the interview if you are saying that you cannot go out on your own then you are unlikely to score for following a journey as a prerequisite for this is that you can leave your house.
You should be able to request a copy of the assessment report by contacting the DWP office dealing with your claim, I would leave it till the end of the week to do this.
A Decision should be 2-6 weeks.
