I've had my PIP award review through just over a year early,and i was expecting tick boxes as part of the form but mine is just a smaller version of my original form. i thought i would ask for help with filling it in as its very confusing(well it is for me lol) I took it to the place where i had my pain management course in Milton Keynes,lovely lady took one look and said she had never seen one like this before so i am wondering if anybody has had one similar to mine,also on form it says they only want paperwork ie consultant forms. hospital appointments, etc from 2016 thats when i had my pip consultation.
