Hi
I recived LCWRA in my decision letter this week so many thanks to the site and to you for answering all my questions over the last few months.
Im just wondering is there any need to for me to request a copy of the letter on how they came to their decision for example what descriptors scored points? Im thinking probably not as i dont need to appeal or do you think I should request it for future reference?

It's up to you, requesting a copy will have no impact on your claim, it will allow you to understand how you were placed in the Support Group which might be important at your next review.

What's the name of the form I need to ask for? I don't want the whole medical assessment really just the form that says how they came to their decision. I received this through universal credit by the way.

I'm not sure if the names are the same for UC, for ESA it would be an ESA85A so try asking for the UC85A, failing that just ask for the assessment report.
