DLA ending, told to telephone them

You can ask for a PIP1 to be sent but you will need to provide reasons why you need a paper copy and cannot complete the form over the phone.

I have to warn you that everybody who has requested a form has had problems in receiving it, be aware your PIP claim does not start until a completed PIP1 is received by the DWP.
