Thanks Gordon, I will do that. It's just that I don't ever remember filling any form in before, might be just me forgetting.
If I don't hear anything, I can either phone or I do have an address to contact my local ESA office.

I would start by phoning, if they can't fill the form in while you are on the phone then they will send you a paper copy.

Have just received mt mandatory recon award letter.
Well, it's ok I have been reinstated for higher mobilty and standard daily living.
However, the point scoring is very odd. It looks like some boss has handed my case to some office junior and said "look, find 12 points for mobility and 9 points for daily living, give it to him and as long as they add up I don't care which way you do it".
Yes, you've guessed it: I have been given nearly all the points on things I didn't really bring to light or make a point of and mostly the things I highlighted have been ignored.
I suppose I've got the award so just file it away and forget about it.
I've heard this sort of thing from other members so wasn't really surprised.
