Hi Gordon, I am after information about PIP &ESA [2017, will they still have this info as both were successful] Do you have a link to the the latest template to request information or is the old one which I still have [December 2014] ok to use and send, when complete do I hand it into my local jobcentre ? thank you once again.
You could start by phoning the relevant DWP offices and making the request, if they can't help you then you need to make a Subject Access Request for the information.
