Hi, I am just in the process of filling in the PIP form and I downloaded the 'Random Sample Pages' but the question are asked different? Inasmuch they are a, b, c, etc answers to fill in but on the actual PIP form I've been sent its a lot of yes, no, sometimes, and then filling in examples.

Sorry if this sounds a bit gibberish but its the only way I can explain it.
