Hi I have been called in for a medical assessment but I haven't filled in a new assessment form my last one was 3 years ago .my husband rang them to which they replied that they don't always require you to fill in a new questionnaire. I wondered if anyone else has had this I'm worried that its wrong grateful for any advice
There is no legal requirement for a form to be issued as part of a PIP reassessment but you are the first person who has reported not receiving one.
You could write a letter explaining how you are currently affected by your conditions and in particular the problems that you have with each of the PIP activities, you can send this to the assessment centre and I would also send a copy to the DWP office dealing with your claim.
