hi, if you are assessed by an occupational therapist (after a social worker refers them to you). Does their findings and equipment fitted list then be sent onto your GP and be put on your health record?

From what I remember, it was some time ago, they did not contact my GP surgery although there is nothing stopping you supplying a copy of your OT report to your GP.
