I claimed ESA at the beginning of April and attended my appointment at the job centre when I was told I would probably be sent an ESA 50 form. 13 weeks have passed and I havnt received a form. I am receiving ESA at the same rate as JSA but no one seems to be working towards assessing me and, hopefully putting me in the support group and increasing my money. I could print an ESA 50 from the internet and I have medical evidence but where would I send it without a reference ? I seem to be in an impossible situation. Any suggestions as to how I can move matters along?

You should have received an ESA50 about five weeks after you started your claim. I would start by contacting Maximus as they are the people that issue the form.
