Hi I need to send my was renewal form back but I have been given two different addresses to return it to. Envelope that came with the form has centre for health and disability, london bsc, mail handling site a, Wolverhampton, wv98 2es. But I got a letter today to remind me to send form back in time with. London, centre for health and disability assessments, London bsc, 333 Edgewater road, london, nw96td. The reminder letter says send to that address so I've no idea which is correct.
I'm afraid you will have to contact the DWP to check. One of the other members recently reported that she had told her form had not arrived because it was sent to the wrong address (even though she was using one they gave to her) - so to be on the safe side I would double-check with them.
