I’m so grateful you posted this because I would have sent the form to the wrong address. I’m going to call Maximus and the DWP for clarification though.

I just spoke to CHDA/Maximus about the different addresses and was assured the form needs to go to the address on the return envelope, which is:

Centre for Health and Disability Assessments,
Birmingham BSC,
Mail Handling Site A,
Wolverhampton,
WV98 2EH

There is a 1st class freepost option. Simply put at the top of the address:

Freepost RTZY-HHBL-YJCZ

Personally I’m sending mine next day, signed for and tracked. As BIS has mentioned the mail handling site does receive mail on Saturdays but it won’t go to the necessary department until the next working day. I’m getting my form in before the weekend.

For peace of mind I’m glad I called.

I only rang maximus because I was expecting a call on my mobile and used the landline cordless phone to ring DWP but i was on hold for so long the battery ran out.
