ESA form not been written in capital letters was created by diane858
Hi Gordon,
i am hoping you can help,
I have been to the citizens advice with my ESA50 form But i hadnt noticed till i got home and checked that it says Capital Letters?
The lady has just written and scribbled everything in and not very neatly,
she didnt really seem to know what she was doing and im a panicky mess so i didnt realise until i came home,
Should i still send the form? i have written on the back of it that citizens advice have filled it in for me
i am having a breakdown worrying now:(

This is about legibility when the form is scabbed or copied, so do you think those assessing you will be able to read it?
If not then you can download and print a new form from

Dear Misty,
Oh no not another one! I've had this problem & so has dear Porthole Pete! They are rushing to fill in the form in the little time available. Some of the CAB volunteers are not very skilled( but at least its someone trying to help?). Try not to panic.
I managed to download a new form & copy most of what had been written using the computer & print it off.I don't think it is dishonest to copy the box where it asks if you have had help filling the form in & by whom. I only just got the form off on time recorded delivery VITAL & keep a copy. If you say you are slightly delayed because you have had to wait for a CAB appointment to get help & advice to fill in the form this should be accepted but this is risky.
I have to say in my case it was the GP letter & Medical Evidence I provided that enabled me to stay in the Support Group. Some of the assessors don't believe what is written on the form & they are unlikely to have read it before an assessment.
I'm sorry you are having to go through this worry & stress.

Hi Maggie,
Thank You, i only went to CAB as i wasn't sure how to explain one of my health issues and i thought they would be much more knowledgeable but this lady had no idea at all and was just reading the CAB guidelines on a pc,zge was a nice lady just no idea,
she hasn't signed the back or said she has filled iin the form but i will put the CAB address on and explain it as she did scan it to their system so i have a copy, i keep trying to ring them but to no avail and they won't answer voicemails,
Its just so upsetting and stressfull as i thiugh i was getting help:(

Good luck with form, I wasn't sure if you had a printer. I'm not sure how you are doing for time to get the form in but Just to say Gordon, Gary & BIS are absolute stars and are really good/experienced at how to explain your limitations to fit the B****** descriptors!
If you post on the forum whats been written about that condition (you can ask for it to be confidential) they are really good at rejigging it. You could then attach an extra sheet to the form if needed. I also sent a photo.
