Hi diceman24

You should add a fit note to your online Universal Credit account at the start of your claim via your journal. You'll be sent a copy of the UC50 form after you tell your work coach about your condition. You can also download the UC50 from GOV.UK if you need another copy.

The UC50 is divided into two halves; activities 1-10 are for physical issues and 11-17 are for mental health ones, you can receive a full award for either half.

Our UC/ESA Claim guide explains the criteria you are being assessed against and how to explain your limitations. See

www.benefitsandwork.co.uk/help-for-claimants/esa1

If you have more questions then please return to the forum and we will do our best to help.

Hi diceman24,

my GP said the longest sick note he could give me was for 3 months. I assume yours will be the same :)

Hi dinkmack

I cannot recall seeing a fit note longer than 3 months, members on the forum may know otherwise.

Hi, not for UC but for legacy ESA, my sister's old GP used to write 6 month FIT notes. But despite her mental health condition not altering for literally decades, when the GP retired a couple of years ago, the ones at the practice she was moved to flatly refuse to do more than 3 months. [What a waste of taxpayers and precious NHS time and money, but common sense never did rule bureaucracy...]. It's a real pain for her with her problems getting round to anything.
