Hi, I have to fill in my review forum and my anxiety and depression is terrible. I feel sick every day due to the though of all this. I only got standard daily living last time. I have fibromyalgia and have just been diagnosed with b12 and folate deficiency. I walk with a limp and now have muscle weakness. Can some one please send me a sample questions I could fill in. Thanks

Hi Kathleen

Welcome to the forum, you might want to have a look at the following FAQ which explains where everything is

Welcome to Benefits and Work

We do have some excellent guides to help you with your claims in our 'Members Only Guides' section. which helps with fibromyalgia part way down the page.

www.benefitsandwork.co.uk/help-for-claimants/pip I assume that it is PIP we are talking about?

If you have any specific questions then please reply to this post and we will do our best to help.

Hi Kathleen. I have fibro, along with other health conditions. Luckily it does not affect my mental health but reviews still make me anxious! I found completing the daily diary a great help, there is even an example for fibromyalgia. I completed it for a week and it really helped me to give examples of what I couldn't do when I completed the form. Not directly PIP review related but if you haven't already, get a referral or self refer to your local occupational therapy team as your life might be improved by aids and they can help. Good luck, still waiting for the result of my review (phone assessment was in July and it was less stressful than face to face)

Hi Kathleen. if you have limited time to get your form in, it's worth if you can manage it, to start a daily diary now, noting how awful you feel. You can send in the form saying you will send further evidence, carry on the diary so you have say a whole month, and send it in after the form. Everything you send be sure to keep a copy and send recorded delivery or similar and keep the proof.
Hope this helps, our daughter feels just like you in terms of depression stress and anxiety and also has fm.
Best wishes, Denby
