Should we notify PIP of deterioration in condition

My husband receives PIP. Due to complications with Post Op Day Case surgery, related to his condition, he has been ,and continues to be unable to work as a self employed person. There is every possibility this will result in Loss of Livelihood.
Should we notify the DWP of this, as we have had nothing but bad experiences post any contact with them. We would be looking for more assistance, but not another meeting at the Job Centre. The last time this happened, the employee threw his arms in the air, and blew a raspberry at us.!

In the DWP guidelines, they specifically say that they should be notified of any change in a claimant's condition. As soon as you put in for a change of circumstances (which is the official term) - your husband will face reassessment. There are still no face to face assessments - they are being done over the telephone or people have been told they will have to wait until face to face assessments resume.
