I'm applying for a new descriptor from my last review form. I had these problems last time although I didn't understand what they were related to and I also have a new diagnosis which relates to this descriptor.
Do I explain this, that I understand what these problems relate to now. It would be incorrect for me to say it has got worse when I already had these problems.

I hope I have explained this well enough.

I would still describe it as a worsened condition, you want them to pay attention to the limitations that you are now describing and if you say that there has been no change then they may not do this.
