I don't think there is a right way or wrong way to do it. I have helped people do it in all the three ways that you have mentioned. I wouldn't try ad put much in the box if you are going to add additional sheets, because it's unnecessary. Just make sure you have a good system for collating it - so it is very clear what is where.

Your friend can request a paper-based assessment, but without backup from a medical professional saying a telephone assessment would cause them overwhelming psychological distress it is unlikely to be granted. (They sometimes even refuse with medical backup). I have only ever known one person to be given one without the back-up and that was only after a fight.
