Letter to GP & HPs re evidence for PIP Claim was created by Hil
What is best way to ask GP & HPs for written evidence in support of my PIP claim? Should I send them a Y/N descriptor list, list of the 13 activities etc? Any advice pls.

There is a GP Notes document in the PIP area which you can use but you should speak to your GP first to understand the support they are willing to supply.

Hi Gordon
They are willing to supply the med. evidence to support my PIP claim, but they are not familiar with how PIP is assessed.
I don’t want to overwhelm them with too much info but they are requesting info. to help them write the letter.
Shld I send them descriptors, activities list, and stuff about what ‘Reliably’ means ? Any advice welcome pls.
