Hi angela
There could be a number of reasons as to why they sent you a letter, they may want to ask you when is the best time for an assessor to contact you, they may want to check if there has been any change in your circumstances, i.e has your condition deteriorated or has it got better.
You will only know once you make the phone call. Do you have someone who can be with you when you make the phone call?
Have a glass of water close by in case you get a dried throat. Have paper and pen handy to write down any instructions, make sure the pen works.

A friend phoned with me this morning.
directed to another number. Then told I need to reapply!
Form "should" be sent to me in 2 weeks.
Now worried about that! Going through all that again. And because I couldnt face upto calling (letter dated 08 sept) I have lost time to prepare to do the whole form all over again.
I had a mental breakdown 2 years ago as it went to tribunal. It seems so much has changed.
I dont know how I will manage a telephone assessment as my breathing is so poor, its difficult to talk and breath if you get what I mean! Already feeling exasperated and fearful.

Hi Angela
Try and take it one step at a time. Would your GP support you and give you a letter stating that a telephone assessment would cause you "overwhelming psychological distress," because of your condition? If they can - you could attach that to the form and ask for a paper-based assessment. The DWP don't always take notice of such letters, but it may be worth a try. They need to use that term.

Hi
my partner took time off work and phoned.with me. And was directed to New Claims.
They took address, did not ask to speak to me, said they would send a form.
They DID NOT send form. I plucked up courage to phone today, I was excessively breathless, barley able to talk. First the woman said I need to ask a series of questions before she is able to send a form. It became clear I couldnt, speak well due to difficulty breathing. She said well phone back with a friend then. I just broke down had to take inhalers before able to continue.
I asked for a reference number, her name, She stated no reference number, and that no record would be made of my call. Again I was struggling so much to breath, and how was i supposed to cope with process. This call took 17 minutes! Eventually she asked me to wait while she went off, on return she agreed to just take my name and address, and slowly I did.
Woman agreed to post form! But its anyones guess. I honestly dont know how I am possibly going to cope all before 14 December. Let alone cope with a telephone assessment. Seems to me they are deliberately making it difficult to even get the form for review/re application.
Have you had any reports of such difficulty getting form.
Whats level information needed to review aka new application?
How can people with breathing difficulties possibly cope with telephone assessment?
My award was granted at a tribunal 2years ago things have got worse. But I have less people involved in my care, COVID being a big reason.
My anxiety, depression, got so bad last time I planned to end my life. Now this, I really dont know if I can do this again, and with limitations of covid, making getting medical appointments/evidence can be more difficult? Help please!

The form you have asked for is a PIP1, it asks the same questions as when you originally made your claim for PIP, there is a list of them in our PIP Claim guide.
In all honesty, I would get you partner to phone again while you are available, the form should take less than 15 minutes to complete.
You can wait for the form but a significant number of members who have requested it have had problems receiving it and that was before the pandemic, your new claim will not be considered as having started until the completed form is received by the DWP.
