Nothing on this board constitutes legal advice - always consult a professional about specific problems

Hi Anne
It's impossible for us to tell you what to say - because we don't know you, your medical conditions or your circumstances.
The planning a journey - you don't need to actually take a journey - it's whether you could plan to get to the shops and back. (eg - you would know how to get to the doctor's). If you struggle with how you would do this - you would need to say.
Jot down all the issues you have with mobility. Look at which criteria you think they fit into. Say why your mobility has got worse and make it clear whether they are physical or mental difficulties or both.
Don't let an assessor force you to say anything. Tell them you need a drink of water if you're beginning to feel you will say the wrong thing.

Could you, using a local map, work out the directions to get from your home to your GPs? If not why not? The assessor will be expecting a cognitive issue or a sensory issue such as blindness to prevent you from planning a route.
