pip review form

hi mods, i have received my pip review form to complete and have today been given a 2 week extension what i am asking is even if my answer would be no change i want to say i still have problems and still need supervision to do the activity like preparing food and cooking , should i write this in box 1 tell us if something has changed and when, box 2 tell us how you manage this activity now , or box 3 tell us about any changes to the help you need from another person. and do i then go on with sending lots of extra pages about how it affects me as i did in my original pip claim 3 years ago or will they have them to read from my last claim, i dont want to send pages of extra information the same as last time if they will have it in front of them and i was happy with my award except for the part about managing toilet needs as i believe i should have been awarded points so i am going to send medical evidence to back this up. i hope you understand the question the way i have written it, many thanks bb
