Hi Im trying to find a blank PIP form to complete for a 16 yr old type 1 diabetic with an insulin pump to make a claim as DLA has now stopped. Not having much success with this. Is there a link for a simple blank form so we can complete it with the guides and know where we are starting from with these telephone calls? I can't find it on the site.
