ESA Review form

Gordon, I have not, touch wood, been sent my ESA review yet (red herring the other week), but when it comes I want to print the form off so I don't have to touch the one sent. Another moderator said to make sure it is the review form I print off. Firstly am I right in saying this will be the ESA50 plus as you have been doing this a long time, can I check that the DWP in your knowledge have never rejected a ESA claim because a claimant has not used the form sent but instead printed one off themselves?
