Hi, currently in support group( since 2011) but just received an appointment for telephone assessment. No forms sent to me so last paperwork was submitted to them over 3 years ago. Are they allowed to do medical with only paperwork from so long ago? Also, I have asked for it to be recorded and they said they will put my request in , I know it was my legal right to have it recorded in face to face assessments but can you insist a telephone assessment is recorded?

1. Lack of review form. Yes, it has been quite commonplace for them not to send in a form. However, if your condition has changed or there's things you want to say, you should ring and ask for the form. Don't let them fob you off, if you want to give more information. Yes they can just look at past paperwork and some people would be satisfied with that. I can't give you a definitive answer because it will depend on your medical conditions, where you are at and how much additional information your might want to or should give to them.
2. Recordings: Yes you are entitled to have your ESA assessment recorded even if it is over the phone.
