universal credit uc50 heath assesment form was created by Foz
hi would like help as just received the uc50 health assesment form currently on pip enhanced rates why do you have to fill thease dreading forms out its just as bad as a pip form aiming to be on lcwrg as unable to work any help and advice would be great.

Hi Foz
It can seem quite daunting completing these forms, my advice to you is to take your time, if you are struggling, do one question a day and follow our guides. Remember UC descriptors are different from PIP descriptors although a few questions do crossover, there is a spreadsheet in the guides which show which questions crossover.
The UC50 is divided into two halves; activities 1-10 are for physical issues and 11-17 are for mental health ones, you can receive a full award for either half.
Our UC/ESA Claim guide explains the criteria you are being assessed against and how to explain your limitations. See

To be placed in the support group/LCWRA, you have to pass two tests, information is in our guides.

Hi how does the different routes to get in to the support group work and the exceptional circumstances work would it be qualified if you were agoraphobia alongside outher medical problems.

All the information you need about the routes into the support group are in our ESA guide on pages 21 - 29.
UC & ESA resources
If you have any specific questions, please do ask.
ESA / UC is assessed on how your condition affects you, rather than on what your particular condtition is. When you complete the form, take special care over all the mental health questions (11-17) and think about how your agoraphobia affects you. Clearly it impacts your ability to go out, but if you are forced into going into a work place, would it affect your ability to behave appropriately etc etc.
