I don't think it is the decision makers report you want, but the assessors report.
You have not said for which benefit you are talking about?
If it is the assessors report then:
For PIP the assessors report it is called (PA4)
For NS ESA the assessors report is called (UC85)
For ESA the assessors report is called (ESA85)
If I have got it wrong please come back to the forum.

Thanks Gary. It is however the decision makers report I need.
I got the assessors report so had to write to the decision maker correcting many of the conclusions he came to. I was then awarded enhanced on both for an ongoing period.
This was a year ago now, but I think I should know where the decision maker agreed with me.
This was for PIP.

You need to contact DWP and ask for a copy of the decision makers report, I am not aware that it has a specific name.
