I have recently won my appeal thanks to you and all your help and advice. I would like to ask the DWP if i can ask to see the notes when i had my telephone assessment to see what they put on there. Would i be allowed to ask for these? And is there a template letter i could use to send to the DWP to ask to see my notes. Thank you.

The notes taken during an assessment go onto the PA4 form. If the appeal you mention was a Mandatory Reconsideration it is possible that you have already requested this. If not, then you simply write to the DWP at the address at the top of your PIP letters saying 'please may I have a copy of the PA4 form with respect of my PIP assessment carried out on XX/XX/XXXX. Or words to that affect. You can also ring them up and ask for the same thing.
If the appeal was to a Tribunal then the PA4 should have been included in the bundle of papers you received prior to the hearing.
