Hi looking at the universal credit form to apply, where it says put in consultants/doctors and people I deal with for help there doesn't seem to be enough boxes, do I sent the info on a sperate sheet? I will print it up and add them together? thank you for your advice.
Hi sam, Of course if you need more space, add a page, and fasten it securely to the rest of the form. Make it clear you have added a page, and VERY important, make sure yourt name and NI Number are clearly on the top of each new page. I hope this helps. Good luck with your claim!
