Requesting Assessment Report was created by stephopal
After successfully retaining PIP after Paper based review. I decided to call PIP to request the report to file for my records.
Still not received as yet.
When an assessment is completed by DWP decision maker do they compile a report, or is it only when it goes to HCP assessors they compile PA3 report?

Hi Stephie
I am not an assessor but I have always assumed that the HCP assessors produce PA3 reports along with a PA4.
PA4 reports are normally uploaded after 48 hrs of an assessment, I would assume PA3's are in a similar time frame but I do not know for certain.

Thanks for your response Gary.
So it’s HCP that complete both PA3 & PA4 reports, who then sends onto DWP, but if DWP decision maker completes your assessment on paper without involving HCP assessor, then there’s no report only their decision statement, they include with the decision letter.
Is this correct and would explain no report being available in my situation?

Hi Stephie
The PA3 form is when a paper based assessment is done.
When it comes through the IAS/Capita queue, they will check whether a Paper based assessment can be done with the information they have, and sometimes they can get additional evidence by phoning the claimants Doctors/Health care providers, CPN’s etc.
A PA3 form is then completed and sent to DWP decision maker, to make a decision.
