Hello,

I have just been informed that my medical information is under review for limited work capability and I need to complete the dreaded UC50 form and re-submit evidence.
The problem with evidence is that all my medical letters are online as my hospital operates a 'paper light' system. Also, the relevant department 'lost' all my original letters after my last assessment two years ago.
I am incredibly anxious about this situation. What should I do?

Hi Rachel,
I appreciate that this may be a worrying time for you try not to let your anxiety overcome you.
I wasn't sure if you meant the hospital had lost your records, or it was DWP who lost them. If the hospital still have the records you could ask for them to send you a copy via email. Otherwise you are entitled to make a request for a copy of your medical records, you could ask your GP for this. There may be a small charge for a hard copy. If you manage to get a copy, make sure you photocopy or scan this before sending to DWP and keep this copy at home for your records.

Hi LL26,

Thank you for your kind words, I am feeling incredibly anxious about the re-assessment but I am trying to keep it from running out of control.
It was Maximus who lost my letters after they were posted to them as part of my last review. I think it was an oversight on the behalf of a lovely woman at my local CAB who helped me with the form.
I have letters in digital format from the last 18 months from the hospital, and an abundance of photocopied letters so I’m hoping this will be okay.
