Hi I have had a dreaded call from my Universal Credit work coach, since my health has declined I have supplied a sick note for 3 months, I already get PIP standard rate. all seemed to go okay, there has been no mention about filling in a UC50 FORM, should I be doing this? very worrying, the coach was trying to push me to get re assed on my PIP to get the higher rate, I said I dont have the energy or help to fill out the form yet, made worse by being Dyslexic. and covid lockdown. Thank you, for all your help.

Hello Sam,
Hopefully you found your coach supportive. You don’t need to worry about filling in the UC50 immediately, but you will be sent one. When it arrives you have four weeks to get it back to them. If you want to start thinking about it in advance there is a link to a form here.
UC50 form
You cannot send it to them until they have sent the paper form to you.
If it is easier for you to fill in an electronic form than a paper one you can use the one I have just sent you a link for. Just remember to keep saving it as you work, and you will need to print it out and post it when you are done. It is a little bit early in the process to be asked to complete the UC50 form, but I am sure that when the time comes your work coach will tell you.
As with your PIP application it is important to send in as much evidence as you can, you can also use this time to start collecting it.
Finally, do take time to read our guides to help you make the best possible application.
