Hi I’m going utterly mad trying to get my go to do an accurate fit note.. I was on esa support group and been on disability benefits since 1999, I came off them 3 years ago as I got an inheritance but recently had to apply for universal credit.I, having a fit note nightmare,. My gap keeps getting it wrong, amd I’ve had so many wrong ones. I just need 3 things.. the correct start date which he keeps getting wrong, my most disabling conditioms, which he keeps getting wrong,. Putting conditioms I don’t actually have on, missing out my most serious ones, he missed out ‘rhumetoid’ so just put arthritis, missed a life threatening condition out, then downgraded my severe mental health diagnoses to ‘ emotional problems, depression and anxiety’, and now he’s put a 6 month end date as if ill be fit for work, when im seriously ill and unable to work.. im at a loss as to what to do, I feel like he doesn’t understand my conditioms , he never reads consultant reports, etc. I feel like im blue in the face asking him to leave the end date blank, and get my conditioms right..
when I did pip, he wrote such a rubbish letter he wrote that I had so many conditioms he would not list any of them here, amd they didn’t look at the rest of my evidence and I had to fight a mandaroty reconsideration to get it, amd I can’t cope with that happening with UC.. I’ve written so many times emails to him, but he just doesn’t get it, I now have about 6 fit notes, all wrong in some way and feel hopeless even trying to get in the disability bit universal credit

I'm sorry you're having such an awful time. I'm afraid there is very little I can offer here in the way of a useful answer.
Have you actually sat down with your GP? (I know that is impossible for some in these circumstances). You are not likely to get very far unless you can either see him face to face or have a telephone consult or video consult. I don't know whether your GP offer either, but it's much easier than email. If they agree you need to write a very clear list of what you need. Don't waste any time criticising what they have done in the past because you will just get the GPs back up. Try and tell him in the politest of ways that you are in danger of not getting benefits to which you would be entitled to with wrong information. He actually doesn't have to list all your conditions - you want the main ones that affect your ESA and he can always say there are others. That will be enough if the main ones are there.
Try to get your GP to have another go. If that doesn't work - do you have anyone who can speak on your behalf. An intervention from a relative or a family member might also help. You only have to send a note to your doctor's giving permission for someone else to speak on your behalf.
