I had not heard of LiMA so had to do a bit of digging
In answer to your questions:
1) Assessments are not yet recorded automatically, we recommend claimants make a covert recording, see page 93 on our PIP guide. If you ask your assessment centre to record the interview then it could delay your claim.

The assessors - who are nurses, doctors or physiotherapists - are guided and prompted by a computer programme, designed by Atos in conjunction with the DWP, called the 'Logic-integrated Medical Assessment' or 'LiMA'.
I do not know if this is still being used.
3) I believe that the Health Assessment Advisory Service is another name for Maximus which is a private company.
