Hello, I've just had a PIP review and received my PIP award letter. I did not have an assessment with a health professional and the decision was made based on my paper evidence. I would like to ring up and request a copy of any report done by the decision maker but I don't know what to ask for. When I had a face to face assessment I asked for a PA4 but is this still relevant if the DM has made a paper based decision and no health professional was involved?
