I don't know whether this is an actual letter that goes from the DWP to Capita - or whether it is just a tick box form. They are your notes so you can request to see a copy of them. It shouldn't affect your assessment phone call - because frankly, they are not likely to respond to your request very quickly.
Obviously, you have your own reasons for making the request but the majority of people are assessed at review time (even those that have evidence their condition will never or is unlikely to improve).
