MR form? was created by Judy

Do I need to request and use the CRMR1 form? I ask because I've forgotten to request it but I've done an MR reconsideration letter with proof of meds and consultant letters. Can't I just send it in without the form saying it is a request for an MR decision please. I've worked so hard on it.

Hi Judy

You don't have to use the CRMR1 form. Some do, and others don't. Just make sure that you've put in a request to submit an MR.

Hi Judy,

If you want to use the CRMR1 form you can download it from here - assets.publishing.service.gov.uk/governm...sion-made-by-dwp.PDF

As BIS says you don't have to, I never do, just go for what suits you best.
