Hello everyone, I'm new to this site.
I have a question...
I received a pip 1043 form and my anxiety has now reached overload.
Specifically its because I have absolutely no memory of what was on my original form back in Oct 2017 and no recollection of which areas I gained points. My disability is mainly mental health. Bpd, Ocd, depression and anxiety. Having said that I was also diagnosed with fibromyalgia in 2018.
Anyway I'm in an absolute frenzy of how to fill this in. As I don't know when or how things changed for me.
All I know is that my health mentally and physically has declined slowly since 2017.
Any suggestions of what to do or say would be so appreciated.

Hi Susan

I'm sorry that you have found yourself in this position. We always advise people to copy any forms or additional information that is sent in.

I would advise that you look at our guide to PIP claims and reviews. The review information is near the end of the guide and it will help you to fill in the form. There are some examples of the sort of answers you may want to give.

If you can't remember exact dates then you will have to guess and say because of your mental health difficulties this is what you have done. You can only do your best and give them the clearest picture you can in the circumstances.

You could put in a right of access request and get a copy of your 2017 form. You could also ask for a copy of the assessment report to see what was said. This is a PA4 if you had a face to face assessment or a PA3 if you had a paper-based assessment. However, this will not be done quickly so you are unlikely to get the information before you have to complete the form.

if you want to try and get access to your previous information you could tell the DWP that you have no records and ask them to give you an extension.

Got it sorted. Phoned them up and they sent me a copy of my original claim. All filled in now. So the waiting begins
