I see that you posted just over two months ago to say that you were about to send off your review form. This is the same form as you will be given if you notify them that your condition has changed. I suspect that you have not yet had your assessment based on that form, so I would recommend that you tackle the problem very differently. Just send in ‘additional evidence’. This can be a letter from yourself or someone who knows you, saying how your condition has deteriorated and how it effects your daily living and mobility. This additional evidence will be added to what you submitted a couple of months ago. We cannot give you a template for such a letter since it is very personal to you.
