The short answer is No, you will not have to have another medical and assessment to claim UC, although you will be re-assessed at sometime in the future.
You can claim UC as a top up to your CB ESA, most people claim UC for the Housing Element if they pay rent, you have to remember that UC is a means tested benefit, so any income you receive or a partner 's income if you have one, will be taken into consideration £ for £ www.gov.uk/universal-credit
