Hello gillybee,
I always advise people to include as much evidence as they can, regardless of whether it is a first application or a review. I am sure there is a technical difference between a report and a letter, but I very much doubt whether a busy GP is going to think about which is which, particularly if he/she gets the message second hand from the receptionist! I would suggest that you simply ask for the GP for a supporting letter detailing your son's conditions - which sounds pretty much like a report to me
