Hello Lynn,

You can go with just ‘no change’ in every box if that applies to you.

However, if you have the time and energy we recommend that you give fuller answers. E.g. No change – I still cannot safely hold a knife to peel or cut vegetables and I am unable pick up a small saucepan filled with water.

When I help people complete that form I very often type the answers on a separate sheet, because as you say, the boxes are very small. I then just write in each box ‘see separate sheet’. If you decide to do that make sure you put your name and NI number on the top of each sheet, and it is good if you can also put page numbers, e.g. ‘page 1 of 3’.

If you go to our PIP guides you will find a section on how we recommend you tackle the review form. You will find there are two guides – but the section on the review form is in the same in both.

I hope that helps
