PIP REVIEW FORM

I am in the process of trying to complete my review form and have downloaded both of the guides, however neither work with my form. There is no date in the bottom left hand corner of the pages. In the right hand bottom corner it says PIP.1043, in the left the reference 001127/000004/000278, however on the very front page there is also in the left bottom corner 47836-00278-001-001-1. Also there are no tick boxes for any of the question except for question 12. Is there another guide I should be using please as very confused. I would add that in my last review there were tick boxes.

Any advice/thoughts would be welcome as don't want to fill it in wrongly.

Both versions of the guide have a section on the review. Have a look at the contents for the page numbers. The review is simply asking what has changed and the details of those changes. I’m doing my husband’s review for him at the moment and I’m using the guides to help me, along with asking questions here when needs be.
