Assessment Report : Evidence Considered In Formulating Adivce. was created by RHAN
Hi All
After requesting all reports & supplementaries of PA3/4/5 , I have received a basic PA3 v2(I'm guessing version 2) which was supposed to be done by a scottish guy, yet states a Females name. A Female did attempted the First assessment call but quickly reverted to P.Based, , then a Scottish guy attempted a phone call.
As expected though, just ticks in all the “A” Boxes. No point system just ticks.
Ive not received a decision off the DM yet , but Ive pressed hard for a copy of all HP reports & supplementary information , even sent two written subject access requests in, but only now received this one -a “PA3 v2” after phoning them up and what felt like a fight for it.
The report is inaccurate as expected even getting dates wrong, & half my evidence i sent in isn't even included, so without a doubt I'm heading for MR.
I have two questions though..
1. On the report under “evidence considered in formulating advice” , it states the following “Telephone Call to DA support line for advice on descriptor choices.” Can anyone tell me what that means please?
2. What HP reports do I ask for if both attempts which are 3 months apart had started off as phone call based assessments then reverted to paper based?
Is is all the PA’s ? Ive asked for all PA3, PA4, PA5 as I was unsure on what to request.
So far I've only been sent the PA3 v2.

3) PA3 – Review report form (paper-based review)
4) PA4 – Consultation report form

Thank you for the codes for the reports I've noted them down
I really hope someone can tell me what “DA” represents, or has had similar experience so I know what Im up against, maybe “descriptor advisor”?

First of all your full name appears to be your forum name I strongly recommend that you change this. See

I can't say what DA stands for specifically but it relates to a more senior assessor in a central location who can be contacted for advice in deciding which Descriptors to recommend to the DWP.

The assessor recommends the Descriptors that they think the claimant meets it is down to a DWP Decision Maker to allocate the points. If you look at the PIP Claim guide then ticks will correspond to points and you should be able to work out a score for Daily Living and one for Mobility.
