Hi Merry

You really have had a bad time with this. You comment that two people could not lose everything - but the second person on the call was there to monitor the assessor, and therefore, they wouldn't have been logging onto the same system that the assessor would have done. Clearly, something went very wrong, and they haven't been prepared to admit what it was.

If you want to request a paper-based assessment, you could ask your GP if they were willing to write a letter stating another telephone assessment would cause you overwhelming psychological distress. You don't need to be making a complaint against Capita - to ask your GP for the letter or to ask Capita to do a paper-based assessment instead of another telephone assessment.

If you're happy to have another telephone assessment, then clearly, you do no need to talk to your GP.

If you later want to make a complaint about what has happened, you can do that separately if you have the time or the energy.

No, you don't need to make a complaint. You can just ask the AP to carry out the next assessment on Paper due to the distress the first one caused you which has now been exacerbated by the report not being produced.

The second person was not assessing you so would have been unable to produce a report.
