template for requested pip paper review due to psychological distress letter was created by bonniedog
hi ive search the forum as best as my limited abilities will alow me to any way
i thought there was a template for requested pip paper review due to psychological distress letter to be signed by a G.P. available on here
but ive had no luck finding it
ive some download files but im unable to open them
it says i dont have the softwear or something
or perhaps someone could just give me a brief note or idea of what to put on it
along with my neck injury and malignant hypertension im struggling with anxiety and depression which in turn brings on intrusive thoughts and violent coughing ,breathlessness and the fear and possibility that this reaction could cause me to have another heart attack ,
so avoiding prolonged uncomfortable stressful situations such as a face to face or telephone review would be in my best interests and the safest option
anyway many thanks for all or any who can help me with this

There isn't a template for the GP to write a letter. All they need to say is that they support your request for a paper-based assessment as a telephone/video or face-to-face assessment would cause you "Overwhelming Psychological Distress." due to your mental health difficulties and the threat to your physical health. Ask them to use the term 'Overwhelming Psychological distress'.
I don't want to frighten you, but be aware the DWP do not have to agree. If they don't agree - then you can appeal and ask them to reconsider.
