Hi Claire,
For most claimants the ESA claim is closed after more than 4 weeks abroad but there are some exceptions detailed here......

Once you have returned to the UK I cannot see anything in the ESA regs above stating that you cannot go abroad for another 4 weeks.

If I may just state though, that you may want to be a bit cautious in the event that a flight cancellation may happen, and if so you would need to either wait for the airline to re-book you or for you to just cough up the money to rush back to the UK to make it back within 4 weeks, so maybe not book it for exactly 4 weeks without any buffer time.
As far as I read the legislation, and what websites say, it's a very strict time limit, and once you are over it ends the claim.
As the UK obviously keeps border entry and exit records, this would be something that they could easily flag up to the exact time of the day that you both left and re-entered the UK I would imagine.
