Can help me please how to get the Medical assessment report of the Health professional pip revew assessment report ( P4A form or whatsoever they call it)

Hi Yesuf

You ring the DWP and ask them for a copy of your PA4 - which is the name given to the PIP assessment report. The number will be on your paperwork
